During your first visit, we will review your dental and medical history forms, and take X-rays of your teeth. Then, the doctor will meet with you to discuss your X-rays,and your treatment. He will answer any questions that you may have about your dental care.

• Your completed patient information form

• Your insurance card

• Your driver's license or photo ID
